How the report is filed

  • Completed test reports are faxed to 937-333-8555 or emailed to Backflow@daytonohio.gov. Source.
  • A complete report must include all identifying information for the device and for the plumber or tester; failed, illegible or incomplete reports are not accepted. Source.
