Cleveland Utilities: backflow testing rules

Who must test
- Policy section on test schedule; DC, PVB and dual check devices have the same at least annual schedule. Source.

Notices and deadlines

How the report is filed
- The tester normally leaves a copy with the customer and submits a copy to the utility; customer remains responsible; utility supplies forms for the tester. Source.
- Install date and location, manufacturer and serial number, test date, tester name, results, repairs and dates. Source.
Tester registration
- No test results accepted without a copy of current certification on file. Source.
- Kits certified annually by a lab approved by the manufacturer; copy submitted to the utility each year. Source.
