City of Mesa Water Resources: backflow testing rules

Who must test

  • Commercial and industrial customers and homes with dedicated landscape meters must install, test and maintain backflow prevention assemblies under Mesa ordinance and state rule R18-4-215. Source.

How the report is filed

  • Recognized testers (General Tester and Fire Contractor lists) must submit test reports to Mesa's backflow portal (SAMS on crossconnection.njbsoft.com, linked as mesaaz.gov/backflowportal) within seven days of service. Mailed, emailed and faxed reports are no longer accepted. Source.
  • Mesa accepts reports only from recognized testers through the portal; incomplete reports are rejected and a corrected report is due within three calendar days. Source.

Tester registration

  • Recognition requires a Tester Application, a signed City of Mesa Backflow Assembly Tester Agreement, proof of tester certification, and a current gauge accuracy or calibration report (Mesa City Code 8-1-12). Source.
  • Recognized testers calibrate all test equipment annually through an accepted agency and submit the accuracy report; a report more than one year old is expired and the tester is removed from the list. Source.
  • Recognized testers must notify the program of changes to contact details, certificate, gauges and calibration, contractor's licence or proof of insurance. Fire sprinkler testing requires an Arizona ROC C-16 or CR-16 licence. Source.

If a test is missed

  • A tester who fails to submit a report within ten business days gets a warning, then a 6 month suspension, a 1 year suspension, and permanent revocation; if the delay causes a customer late fee, suspension starts at the first offence. Source.
  • A finding of violation brings civil sanctions of not less than $100 or more than $1,000 for each violation, and each day a violation continues is a separate offence. Source.
  • Mesa may terminate water service when backflow protection is missing, bypassed, not working, not tested annually, or test records were not provided on time. Source.
